Face rule 1 : Zygomatico-maxillary fracture

-The zygomatic bone are attached to other bones by multiple sutures which are:


1-Zygomatico-maxillary.
-Zygomatico-frontal.
3-Zygomatico-temporal.
4-Zygomatico-sphenoidal.


A)-When one received a trauma to the zygoma, it will not be fractured as it is a very strong bone, so it will be splitted from these sutures( hence the name quadripod fracture or zygomatico-maxillary complex fracture).
It is a common fracture in clinical practice.

B)How to access the posterior displacement of the zygoma.
Can be accessed as follow:
1-Posterior displacement of the zygoma cause disfigurement of the face.
2-Draw  a line from normal zygoma parallel to the horizon reaching displaced zygoma then, measures the distance between this line and displaced zygoma.

 C)How to access the rotation of the zygoma.
 Can be accessed as follow:
1-To diagnose rotation of the zygoma, look to the epsilateral maxillary sinus, if it is become smaller in size if compared to the contra lateral sinus, This means medial rotation of the zygoma.If becomes larger in size, this means lateral rotation of the zygoma.

D)Presence of hemosinus and surgical emphysema.

E) How to fulfill a report:
1- Bring a paper and write the following
-Names of broken bones.
-Size and sites of bone fragments.
-Condition of the globe( ruptured or not), optic nerve(by MRI) and orbital muscles(entrapped or not).
